A Case of Interstitial Pneumonitis in a Prostate Cancer Patient Treated with Bicalutamide

Abstract

|
Bicalutamide is a non-steroidal antiandrogen used in the treatment of prostate cancer, which is known to induce interstitial pneumonitis in rare occasions. We report a case of a 77-year-old man diagnosed with prostate cancer and treated with bicalutamide for 3 months, and expired by interstitial pulmonary infiltrates and respiratory failure. The patient was presented with dyspnea for a month, and chest radiography and computed tomography revealed interstitial infiltration in both lungs. No bacterium was identified, and the patient did not respond to empirical antibiotic therapy. Based on the chest imaging tests and bronchoalveolar fluid findings, other interstitial pneumonitis was highly unlikely. We suspected bicalutamide-induced interstitial pneumonitis, then stopped bicalutamide and treated with methylprednisolone. However, after a temporary improvement, the patient suffered a worsened respiratory failure, complicated by sepsis, and finally has expired.
